• A question fax form. If you wish to ask a question after the audio conference you may also e-mail it to Education@planning.org.
• We will accept questions until Thursday, September 25, 2008.
• A conference evaluation form.

Copy of the CM sign-in sheet (pdf) You must attend for the duration of the event, complete the sign-in sheet, and claim your credits in the online log at www.planning.org/cm.
