Collective impact occurs when organizations from different sectors agree to solve a specific social problem using a common agenda, alignment of efforts, and common measure of success in a structured framework. The key to a successful collective impact model relies on partnerships that understand the work they are engaged in is already a “part of what they do,” instead of “in addition to what they do.” In San Bernardino County, California, agencies are involved in initiatives using this approach to transform the health of the county. During this discussion, staff from the San Bernardino Council of Governments and San Bernardino Department of Public Health, as well as the county's superintendent of schools will share their collaborative approach to improving health in the community by addressing social determinants of health related to the built environment (planning), education, and health sectors. The session will specifically focus on cross-sector collaboration work related to the built-environment sector and its relationship to both education and health sectors. The session will include discussions on countywide vision (regional-level policies), transformation plans (community outreach), Safe Routes to School (program-level plan), the Toolkits program (implementation), and funding opportunities.
